请洪飞前辈帮我看一下活检报告

不好意思,想麻烦洪飞帮我看一下,有关我女儿的活检报告,她到底按中国的分类是属于那一类霍奇金淋巴瘤?

Biopsy Report

Specimen A: L neck mass- level 4/ anterior mediastinal mass

Diagnosis A: mass, left neck leven 4/ anterior mediastinum, biopsy:- nodular sclerosis classical hodgkins lymphoma.

Gross A: The specimen is label "L neck mass level 4/anterior mediastinal mass." Received fresh in saline is a single portion of tissue consistent with core biopsy measuring approx 1.2 cm in length w/ diameter of approx 0.1 cm. Also received fresh in RPMI are three portions of tissue consistent w/ core biopsy specimens ranging in legnth from approx 0.8 up to 1.2 cm in length, each with diameter of approx 0.1 cm. Touch imprints are performed on core recieved in saline. Two of the core biopsy framents are submitted for flow cytometry. Remaining two core fragments are wrapped in tissue paper and submitted for permanent in cassette A.

Micorscopic A histological exam performed: A: sections show needle core biopsies of lymphoid tissue and thick fibrous bands. background of small lymphoctyes, neutrophils and occasional eosinphils are large atypical cells with irregular nuclear contours, vesicular chromatin and prominent nucleoli (Reed0Sternberg cells); occasional binucleation isalso present. Immunohistochemical stains performed show strong CD30 positivity in the large atypical cells, which are also CD15 and PAX-5 weak focal positive and negative for CD45 and CD20. CD3 stianing is positive in frequent small background T cells and is equivocal in the neoplastic cells.

Antibody Panel- lambda/kappa etc

Cytospin Morphology: similar to touch imprint

Immunophenotypic Findings: 1.5% mature B cells, 37% T cells 1.2% NK cells.

很明确，这是经典霍奇金淋巴瘤中的结节硬化型。中国也是按照国际上的WHO2008分类分的。
CD30+,CD20-,CD15+

对报告不太了解，随便说说，CD30阳性，有可能是淋巴组织反应性增生或者T细胞淋巴瘤。反应性增生的话是良性的。查资料得知的。对霍奇金淋巴

瘤不够了解，免疫组化结果不太明白。

CD30是活化大细胞的标记，如果细胞形态是RS细胞，CD30+,那么就是霍奇金淋巴瘤。如果不是RS细胞，那么反应性增生和T细胞淋巴瘤都有可能，要进一步鉴别。反应性增生的CD30+表现为阳性程度不相同，有的细胞表达强阳，有的弱阳，因为它们是多克隆的。结合显微镜观察需要

classical hodgkins lymphoma ,  即时不看细胞也很清楚了。 就是经典HD.

这个是医生的诊断结果。我的意思是说根据免疫结果，还有可能还会有哪些可能的情况。

谢谢洪飞的指导.那么她这个就是前几天你说的属于"结节硬化型经典霍奇金"不是"结节性淋巴细胞"?她不可以用美罗华，但未来可以使用 SGN-35?我因为看见说是"nodular sclerosis"我以为和"Nodular lymphocyte predominant Hodgkin lymphoma"是一个类型,看来应该是不一个分支.我的理解对吗?
